Improving Patient Safety – Louis Appleby’s in-depth analysis of inpatient care


In 2012, Leicestershire Partnership NHS Trust (LPT) invited national mental health care expert Louis Appleby to carry out an in-depth analysis into a series of suicides of patients who had been in our adult mental health inpatient services at the Trust.

We did this because we wanted to have the views of the nationally recognised expert in research into suicides, so we can be sure that we are doing all we can to avoid inpatient suicides.

Louis’ review served a positive purpose by helping us turn the findings from LPT’s internal investigations into Action Plan recommendations for the Trust, with the key focus being improving patient safety. LPT’s priority has been, and continues to be, to properly implement our agreed recommendations and ensure ongoing high standards of care.

It is important to acknowledge that for some of the actions it has been necessary to slightly revise the action, extend the action to other areas, or review the timescale for completion. This is purely to ensure that the actions remain appropriate and achievable and within realistic timescales, and also where applicable that lessons learned are shared across the Adult Mental Health division within LPT.

Significant progress has been made to all sections of the action plan with a number of actions now fully completed. Where policies have needed revising these have been reviewed, updated and the necessary changes ratified, and the amended policies are now in place. Any changes in practice such as carrying out observations in line with our new Observation Policy, and implementing the new handover procedure, will be captured as part of the clinical audit program of work

Progress on this action plan arising from Louis Appleby’s report is monitored via our Quality Assurance Committee and evidence for the actions is collated and populated within the action plan in the same way as for Serious Incident action plans.
