Pathways of Care
An integrated care pathway is an outline or map of anticipated care a person with a specific health condition or set of symptoms may receive within an appropriate timeframe from a multi-disciplinary/ multi agency care team. A care pathway is designed to help the person move progressively through a clinical experience to a positive outcome.
