We know that people want good information about healthcare services, conditions and treatments and we aim to provide accurate, appropriate and timely information for all of our patients and users.
Much of this information is provided by the healthcare staff caring for you. They will be happy to discuss your treatment in detail.
You can also find information on every service LPT provide under Our Service A-Z. All leaflets can be found by searching the Information Library.  Just select the 'leaflets and posters' tag and enter a keyword such as the name of the service you are looking for.

Getting an information prescription

On NHS Choices you can create your own information prescription for many conditions. An information prescription tells you about: • your condition • your treatment options • care services (from equipment to help you get around the house to specialised exercise classes) • benefits you may be able to claim • housing support • self help and support groups You can discuss your information needs with your healthcare professional or with your social care worker, usually as part of your care plan. Rethink - Good Health Guide

Charity Rethink Mental Health has produced a Good Health Guide, which is aimed at helping people with a mental illness get more physically fit. It did this because it was regularly told that the physical health of people with mental illness can get overlooked.
