Outreach Team for Adult Learning Disabilities Service


What Does Our Service Do?

As part of the specialist Learning Disability Service, the Outreach team provides a multi-disciplinary-focused community-based assessment and intervention service.

We work with individuals who have a learning disability who require support due to the presentation of severe challenging behaviour and who, because of the risks associated with those behaviours, pose a risk to themselves and/or those who care for them.

We carry out comprehensive assessments to inform the development of positive behaviour support guidelines, working together with the person, multi-disciplinary team, carers and other services services. We also provide direct support with this if appropriate.

How Does Our Service Work?

People who use our service can expect to receive an initial contact from a qualified practitioner to gather assessment information and gain consent. If it is decided our service will provide the most appropriate support/help, you will have a named nurse who will talk to you and/or your carers about how the team will work with you; whether we need to meet and talk to you, talk to other people or observe how you are. We would gather this information together and share it with you and your carers to help everyone consider if and how changes can be made to help support you.

The team currently consists of eight qualified nurses (RNLD) five healthcare support workers, an occupational therapist and a speech and language therapist, with support from psychiatry. We receive more than 150 referrals (based on 2015 figures) a year, working actively with more than 50 active cases at a time. Last year the team responded to 88 urgent referrals (contact within 48 hours).

The Outreach team is based on The Agnes Unit, our Trust's assessment and treatment inpatient unit for people with learning disabilities. We cover the city of Leicester and the counties of Leicestershire and Rutland. Our office hours are Mon–Sun  8am – 9pm.

How Can You Access Our Service?

To access our service for assessment you can:

  • Speak to any health or social care professional involved in your care
  • Contact us directly and refer yourself, providing you are over the age of 18 and have a diagnosed learning disability
