Adult Community Mental Health Teams


What Does Our Service Do?

The Adult Community Mental Health Teams (CMHT) are made up of Consultant Psychiatrists, Psychiatric Nurses, Occupational Therapists, Social Workers and Psychologists providing a range of treatments, interventions and assistance to adults (people aged 16-65) suffering from difficulty with their mental health who have been referred by their GPs or other health professionals. The types of illnesses people are referred with include bi-polar disorder, schizophrenia, severe depression, personality disorder, dual diagnosis and aspergers/autistic spectrum disorder. People requiring on-going treatment are seen in clinics, appointments made at CMHT bases and through home visits. There is also a developing group work programme.
 

How Does Our Service Work?

A comprehensive assessment will be completed either in outpatients or by the community team which will determine the type and level of need and subsequent interventions and treatment. This may involve psychosocial interventions, medical prescribing or, if eligibility criteria are met, social care commissioning of services. Each patient taken on for on-going treatment will have a care coordinator or lead professional and will be provided with a written outline of the proposed care either in the form of an outpatient letter or care plan. The aim of any intervention being to promote wellbeing and recovery.

  • Mental Health Units
  • Community Hospitals
Adult Community Health Teams in Leicester, Leicestershire and Rutland are currently seeing 6000 patients in consultant psychiatrists’ outpatient clinics while the multi-disciplinary community teams see 2800 patients.
