Heart Failure Service

Heart failure is the name given when you are diagnosed with a problem with the pumping action of your heart.

The Long Term Conditions (LTC) heart failure service comprises a team of heart failure specialist nurses and specialist physiotherapists, who deliver heart failure rehabilitation. The team see housebound patients in their homes, provide heart failure clinics in the community hospitals, health centres and some GP surgeries. The service aims to promote best practice in the management of heart failure and support patients to reduce hospital admission and self manage/monitor their condition. We also help and support patients and carers with heart failure through the advanced stages of the illness and into end of life.

The teams work closely with our secondary care heart failure colleagues to deliver integrated evidence based care.
