Heart failure specialist nursing


What does our service do?

The community heart failure specialist nurse service provides care and support for adults with a diagnosis of left ventricular systolic dysfunction (heart failure) and their families and carers.  

How does our service work?

Our specialist nurses will involve the patient and their family in their own plan of care. The aim is to empower the patient to assess their condition,  understand their treatment, recognise signs and symptoms and understand the significance of any changes in order to take the appropriate action.

We provide care in:

  • patients' own homes
  • GP surgeries
  • community hospitals

  • Speak to any other healthcare professional involved in your care
