Mental Health Services for Older People (MHSOP)

The MHSOP Central Referral Hub – COUNTY WIDE SERVICE

All referrals into Mental Health and Well-being Services for Older People from across Leicester, Leicestershire and Rutland are received by the Central Referral Hub. The hub is based at The Neville Centre on the Leicester General Hospital site.

Once a referral has been received the patient will be contacted by the most appropriate team who can meet their current needs.

Referral Criteria

People of all ages with probable dementia

Adults over the age of 65 with depression, anxiety or psychotic illnesses

How to refer By letter, phone, email or fax
Referrals can be received from

• GP’s

• FOPALS (new referrals and unscheduled care)

• UHL

• Community Hospitals - ANP’s

• ICRS (City)/CRS (County)

• Non LPT Psychiatrist (private patients)

• Transfers from Out of Area (patients already known to services)

The Unscheduled Care Service - COUNTY WIDE SERVICE

This service provides assessment and intervention for patients who require an immediate response. This service runs from Monday to Friday between 9am – 5pm. An out of hours service is provided by the MHSOP On Call Consultants.

Referral Criteria Response to high risk (suicide/self-harm/risk behaviours associated with dementia); prevention of admission where appropriate.
How to refer Via the Central Referral Hub
Referrals can be received from

• New patients to MHSOP from the Referral hub requiring an urgent response.

• Known patients referred from Out Patients and Memory Services requiring an urgent response, after indication by the Lead Professional.

• Internal referrals from OP/Memory Services will be accepted if the level of urgency is such that the referral cannot wait until the next weeks MDT for discussion.

In-reach Team - COUNTY WIDE SERVICE

The In-reach team provides a multidisciplinary assessment and specialist support to patients with Dementia who are living in a care home. The team sees patients who are experiencing difficulties due to their dementia and require specialist support and advice. This team also provides mental health assessment and specialist support to older patients with mental health problems who are current inpatients in a community hospital. The team comprises of Psychiatrists, Mental Health Nurses, Occupational Therapists and Health Care Support Workers.

The team is accessed via the Central Referral Hub or directly by community hospital staff.

Referral Criteria

Patients referred from the MHSOP Referral Hub requiring intensive assessment and intervention

Patients with a diagnosis of dementia residing in a care home – residential/nursing

Patients demonstrating behaviour and psychological symptoms of Dementia (BPSD) and where there is an indication for placement breakdown leading to potential hospital admission. This could be due to a risk of harm to self and/or others/behaviour impacting on the ability of the care home to meet patient needs

Patients require intensive support to maintain placement in a care home.

Management of withdrawal of anti-psychotic medication giving rise to behavioural changes which could impact on placement in care homes

Patients requiring mental health assessment within a Community Hospital setting.

Community Mental Health Teams – SUB LOCALITY LEVEL

Community Mental Health Teams provide more routine (non-urgent/non intensive) multi-disciplinary assessment and intervention for patients of any age with a complex presentation of dementia or those over 65 with a complex functional illness. This intervention takes place in a community setting, for example, patient’s own home. The team comprises of Psychiatrists, Clinical Psychologists, Mental Health Nurses, Occupational Therapists and Health Care Support Workers.

Outpatient Service - SUB LOCALITY LEVEL

The Outpatient Service provides assessment and treatment for people over 65 years of age with depression, anxiety or psychotic illness within an outpatient clinic setting. Clinics are held across Leicester, Leicestershire and Rutland.

Memory Services- SUB LOCALITY LEVEL

The Memory Service provides diagnostic assessment, treatment for people with dementia within a clinic setting, care home or their own home. This service is provided across Leicester City, Leicestershire County and Rutland. The service is provided by Psychiatrists and Mental Health Nurses.

Primary Care Integrated Care Team – Mental Health - SUB LOCALITY LEVEL

The service provides mental health specialist assessment and intervention within a primary care environment for patients over 18 with a long term condition linked to a mental health presentation. This aims to reduce the need for admission to a physical or mental health inpatient bed as well as prevent / reduce the need for referral to Secondary Mental Health.

Referral Criteria

Patient over the age of 65, e.g. those who are frail or who have Long Term Physical Health Conditions, where there are concerns regarding their Mental Health e.g. Depression, Anxiety, Cognitive Impairment.

Patients aged 18-64 who have Long Term Physical Health Conditions that are negatively impacted by their Mental Health, or vice versa.
