Rehabilitation Care Pathway Nurse

The aim of the Rehabilitation Care Pathway Specialist Nurse is to be involved with people on their journey in the rehabilitation service from admission to discharge.

The role includes assessing people who have been referred to the service from other wards, from the community and occasionally people who have been referred who are living out of Leicestershire. However, the role mainly focuses on planning and co-ordinating the discharge of people when they are ready to move on from the rehabilitation service.

The Rehabilitation Care Pathway Specialist Nurse is also sometimes referred to as the Discharge Facilitator or Co-ordinator, and ensures that people being discharged are able to continue their recovery in the community, or identified placement, depending on the individual assessment of each person.

This process involves working with our commissioners, social workers, community mental health teams, housing associations, residential homes etc. as well as with the inpatient rehabilitation team, to ensure that the discharge process is timely and meets the needs of each individual.
