Primary Care Coordinators


What does our service do?

Our Primary Care Coordinators are a team of registered healthcare professionals who work within the Emergency Department at Leicester Royal Infirmary (LRI).  Their role is to identify and arrange care for patients who could be cared for in the community.

The team includes nurses and physiotherapists who work in collaboration with therapists, consultants and geriatricians at the LRI, as well as social services teams.

The team will assess each patient and family using an assessment criteria to ensure that they receive the most appropriate care for their condition. They provide an outreach service for patients who are discharged home. 

How does our service work?

Patients who are referred to the team can expect a holistic assessment. The assessment will identify what services and equipment a patient may need in order to continue to live independently in the community. If the assessments show that it may be unsafe for a patient to go home straight away, suitable care will be arranged.

How can you access our service?

You will be referred to our service if you attend the Emergency Department at the LRI and your care or treatment could be provided in the community.
