Leicester City Transitions Team for Health
Supporting young people who have ongoing health needs as they prepare for adulthood
What Does Our Service Do?
We are the Leicester City Transitions Team for Health. We are part of the Diana Specialist Nursing Service in the Families, Young People and Children's (FYPC) division of Leicestershire Partnership NHS Trust (LPT).
We work with young people (aged 14 -19 years who live in Leicester City and have a City GP) who have ongoing healthcare needs. We provide support to them and their families as they prepare for adulthood and the transition to adult healthcare services.
How Does Our Service Work?
We offer advice, support and co-ordination to young people and their families as they prepare for adulthood and help them with the transtion to adult health care services.
We provide home visits to:
- Establish ongoing healthcare needs
- Involve young people and their famlies in planning for their future.
- Work with young people and their famlies to produce a personalised Health Action Plan which will include information about who can provide the support they need in adulthood.
We can attend Person Centered Reviews at schools.
We offer advice to other profressionals around overall transitions to adulthood and adult healthcare services.
We work with schools, social care providers and other agencies to support planning for transition.

We accept referrals from professionals, schools, young people, families and carers.
We will then make contact to arrange an appointment to meet with the young person and their family/ carer at home.
