Outreach Team for Adult Learning Disabilities Service

Our Outreach Team work with individuals who have a learning disability who require support due to the presentation of severe challenging behaviour and who, because of the risks associated with those behaviours, pose a risk to themselves and/or those who care for them.

Intensive assessment and support for people with a learning disability and their carers and challenging behaviour which is resulting in significant risk or harm to themselves or others. Referral criteria is primarily based around:

1, Prevention of immediate breakdown of community placement due to risks associated with challening behaviour resulting in possible psychiatric hospital admission.

2, Supporting discharge from the Agnes Unit.

3, Supporting transition of people with a learning disability and challenging behaviour of people who are returning to Leicester from an out of county specialist service through the Transforming Care Programme Board.

Referral criteria

Adults aged 18 years + with a diagnosed learning disability and associated challenging behaviours and/or mental health needs.
