Tissue Viability Service

Our Tissue Viability service offers guidance and advice to health care professionals who are involved in the management of complex wounds.

 

Tissue Viability is a specialist service that provides telephone/electronic advice and support to all healthcare professionals who are involved in the management of complex wounds. Face to face joint visits can also be arranged with the health care professional and patient where required.

This Includes: complex non-healing wounds, category 4 pressure ulcers, deep/undermining category 3 pressure ulcers, complex diabetic foot ulcers (in conjunction with podiatry), ischaemic limbs, end of life wounds that are affecting death with dignity i.e. very leaky/malodourous, self-harm wounds, human bites, complex burns, difficult to manage fungating tumours, difficult to manage PEG sites and unmanaged incontinence associated dermatitis

The Tissue Viability team also provides a range of education programmes to support health care professionals to deliver care in line with current best practice.

Referral criteria

The Tissue Viability Team is a nurse lead service supporting patients in the community. They provide specialist advice for the ongoing treatment and management of complex wounds. Referrals are made via a registered nurse or GP.

The referral criteria includes:

Complex non-healing wounds, category 4 pressure ulcers, deep/undermining category 3 pressure ulcers, complex diabetic foot ulcers (in conjunction with podiatry), ischaemic limbs, end of life wounds that are affecting death with dignity i.e. very leaky/malodourous, self-harm wounds, human bites, complex burns, difficult to manage fungating tumours, difficult to manage PEG sites and unmanaged incontinence associated dermatitis

GPs and other healthcare professionals with access to SystmOne should make an e-referral to SPA using the PRISM Referral form or the SPA Referral form.

GPs and other healthcare professionals without access to SystmOne should send the referral form via secure email to lptchs.spa@nhs.net.
