Referral criteria

Referrals for this diagnostic service will be accepted from any Health or Education professional, where there is concern about hearing, speech or communication.

Please note: we are unable to accept referrals from parents/guardians directly.

This service accepts referrals from any health or educational professional and is appropriate for children aged from 7 months to 16 years of age (19 years of age if child attends a special school), where there is concern about hearing or communication difficulties. Parents/carers can request a referral to this service through their nominated health or educational professional.

Referrals can be made using the FYPC Referral Form

Referrals are accepted by post to:

Leicestershire Partnership Trust, Bridge Park Plaza, Bridge Park Road, Thurmaston, Leicestershire, LE4 8PQ

Or secure e-mail : fypc.referrals@nhs.net

SystmOne users can also refer using e-referral within the child’s record, (Refer into the Medical Allied Unit.)

Please Note: Referrers should include information about child’s developmental stage, as well as chronological age, to ensure child is seen on a clinic that is appropriate for them.
