Healthy Together 0-19 (Health Visiting and School Nursing)

Healthy Together is a universal service for children and their families from 0-19 in Leicester city and 0-11 in Leicestershire and Rutland, and includes health visiting and school nursing services.

We offer services for every child and their family – from before the baby is born right up to the age of 19 in Leicester city and 11 in Leicestershire and Rutland. Children will be seen by a public health nurse (health visitor) six times before the age of 2 with health and wellbeing being assessed at key points in their school life (Reception, Years 6, 7, 9, & 11).

Referral criteria

Healthy Together is a universal service for children and their families from 0-19 in Leicester city and 0-11 in Leicestershire and Rutland. If you would like to discuss a referral to the service please ring the Healthy Together helpline on 0300 3003001, where you will be directed to the contact details of the appropriate neighbourhood team.

Here is an overview of how contacts are being delivered at each stage:

Contact point Nature of contact
Antenatal contact You will be contacted by your Public Health Nursing team between 28 – 36 weeks pregnant.

You will receive a letter detailing how to access digital resources on the Health for Under 5s website and ChatHealth secure text messaging service.

New birth review You will be contacted by your Public Health Nursing team, to arrange a contact when your baby is 10-14 days old.

All public health nursing staff will wear the appropriate personal protective equipment (PPE) during face to face visits in compliance with national infection control measures.

6-8 week review You will be contacted by your Public Health Nursing team to arrange a contact when your baby is 6-8 weeks old. Part of this contact involves your baby being weighed.

3-4 month review You will be contacted by your Public Health Nursing team when your child is between 3 and 4 months old and directed to the Health for Under 5s website to view information on wide range of topics, including ‘introducing solids’
10-12 month review You will be contacted by your Public Health Nursing team when your child is between 10 and 12 months old.

How to access this service

All new parents registered with a GP will automatically be invited for their routine public health nurse (health visitor) appointments.

Outside of these appointments, Leicestershire Partnership NHS Trust runs a confidential secure text messaging service for parents of children aged 0-19 years in Leicester City and parents of children aged 0-11 years in Leicestershire and Rutland called ChatHealth. The service operates Monday to Friday between 9am and 5pm, excluding bank holidays. All texts will be responded to by a public health nurse (health visitor/school nurse) within 24 hours. Outside of the service working hours, you’ll receive a message back to inform you that your text will be responded to once the line reopens.

Should you require urgent health advice in the meantime, please contact your GP, visit an NHS walk-in centre or call NHS 111. For emergencies, dial 999 or visit A&E.

Starting and during school years

In the term before your child goes to school parents will receive a letter, introducing the school nursing service and highlighting the support that they can provide to your child in school.

Any child or parent who wants to see a public health nurse (school nurse) can get in touch via their child’s school, or they can text a public health (school) nurse via ChatHealth on the numbers above.

For Young People

ChatHealth, Leicestershire Partnership NHS Trust’s confidential text messaging service, is also available for young people aged 11-19 in Leicester City to use to contact a public health (school) nurse for professional health advice and support.

The service is available 9am-5pm every week day, excluding bank holidays. The text line stays open during the school holidays too. All texts will be responded to by a public health (school) nurse within 24 hours. The service is totally confidential and your information will only be shared if the public health (school) nurse is worried about you or someone else’s safety, or if you share information which may be concerning or put you or someone else at risk or danger.
