Adult Community Therapy Services

Our physiotherapy and occupational therapy teams help people regain or maintain function and independence in their daily life.

Our therapists treat people in their own homes, residential and nursing homes, in outpatient clinics and remotely via telephone or video consultations. Following an initial assessment, the therapy service will work with the patient to achieve mutually agreed treatment goals. The service actively promotes self-management and patients are discharged from the service once their realistic goals are achieved.

Referral criteria

This service is for adults registered with a GP in Leicester, Leicestershire or Rutland who require rehabilitation to support return of function and wellbeing to achieve the best health outcome.

Criteria exclusion

Anyone not registered with a GP in Leicester, Leicestershire and Rutland and those under 17 years of age.

How to access this service

To access our community therapy services you should speak to your GP or any other health or social care professional involved in your care eg. nurse, social worker. Patients can self-refer to the community therapy team by calling the Single point of access (SPA) number 0300 300 7777.

GPs and other healthcare professionals with access to SystmOne should make an e-referral.

Once a referral is received it will be reviewed by a triage clinicians who will call you to discuss your referral.
