Please tell us about the work you think deserves an award using a maximum of 250 words. The judging panel will be looking for evidence of how the nomination meets the criteria for the award and our leadership behaviours, so it is important that you read the criteria carefully. Nominations which are incomplete, or do not provide sufficient detail, or do not meet the criteria will not be judged. Note that the judging panel may include people who work outside of the NHS so try to avoid clinical language, NHS jargon and abbreviations.*

Filming
We ask that the nominators of shortlisted nominees be available for filming (remotely, via MS Teams). Films are shown at the awards ceremony and used on social media. Please let us know if you are happy to be filmed. We will be in touch to confirm whether your nomination has been included in the shortlist following the shortlisting panel in June. We will then give you an exact date and time slot for your recording.
