Online Form available from 5th May 2026
The online self‑referral form for MSK Physiotherapy will be available from 5 May 2026 to patients registered with a GP in Leicester, Leicestershire and Rutland.
Sent a password in your appointment letter?
Until the 5th May 2026, this form is only available to patients who already have a booked appointment with the MSK Physiotherapy service and who are taking part in our pilot of this form. If you are part of the pilot, you will have received a password in your initial appointment letter, which you will need to access and complete this form.
