What happens after I complete my form?
- A physiotherapist will review the information you have provided on the form.
- You may receive a telephone call in the next few days but ONLY if a physiotherapist needs to clarify any of the information you have provided.
- After your form has been reviewed, you are likely to receive exercises or self‑management resources to help you start managing your condition. This will include information about how to book an appointment, if this is needed.
- You will only be contacted directly to arrange an appointment if this is clinically appropriate.
- If your condition or symptoms are not suitable for this service, or would be better managed by another service, you will be sent information explaining what to do next.
How we contact you
Written communication is usually provided by letter. Where consent and contact details are available, this will be sent via email or an SMS link. If electronic contact is not possible, a paper copy will be sent by post.
