Diana Childrens Community Service - end of life care

Our Diana Service provides an on-call facility for children and young people at the end of life.

The Diana end of life on-call service is activated for individual children at this stage of their illness. This service is available 24 hours, seven days a week and is provided by experienced qualified children’s nurses. This care will be provided within the home or within an appropriate community setting.

The service endeavours to offer each family the choice of where they wish their child to die, in order for those closest to the child to have time together.

The Diana Service will endeavour to accept all referrals for end of life care. Due to the complexity of managing end of life care the service is able to accept a maximum of two children/ young people for 24/7 on-call provision at any one time.

Referral criteria

Children and young people up to 18 years old at end of life. Referral by email to fypc.referrals@nhs.net with completed referral form. This can be accessed by phoning in to the service.

None stated.
