- checks that the care and support you receive in hospital and in the community is safe and of good quality
- provides updates to NHS England about discharge plans and quality checks
- attends ward rounds to help plan your care and support a safe discharge from hospital
- works with social care services to find the right community placement and support for your needs, including arranging funding where needed
- helps to find another hospital placement if this is the best option for your care
Referral criteria
Referrals can be made by inpatient teams via a referral through the Dynamic Support Pathway. Referrals are made for Adults, Young people and Children with a diagnosis of a Learning Disability and/or Autism with a health commissioning need for discharge. Not all discharges will require a case manager.
