Looked After Children's (LAC) Team
The Looked After Children’s Health Team (LAC Team) aim to promote the health and well-being of children and young people who are looked after, including unaccompanied asylum seeking children.
An Initial Health Assessment for all children and young people will be undertaken by a Paediatrician, following that, children and young people age 5-18 years will have a Looked After Children’s Nurse and children under 5 years will be cared for by their Public Health Nurse- Health Visitor.
The team supports the child/young person and their carers throughout their time in care with health assessments, packages of care, advice, attending review meetings and making referrals to other services when required.

Referral criteria
Referrals into the LAC Health Team are made by Social Workers for all looked after children.

What conditions do you cover
The general physical and emotional health of looked after children is assessed and onward referrals and support of health is undertaken across the range of health services in primary care, Leicestershire Partnership NHS Trust’s Families, Young People and Children’s Services and acute care where necessary.
