Service description

The Looked After Children’s Health Team (LAC Team) aim to promote the health and well-being of children and young people who are looked after, including unaccompanied asylum seeking children.

An Initial Health Assessment for all children and young people will be undertaken by a Paediatrician, following that, children and young people age 5-18 years will have a Looked After Children’s Nurse and children under 5 years will be cared for by their Public health Nurse-Health Visitor.

The LAC health team supports the child/young person throughout their time in care. This includes health assessments, packages of care and advice, attending review meetings and making referrals to other services as required.

Carers and young people can contact the LAC Health  Team for general advice on 0116 2951370 or make an appointment with their GP for specific health needs or illness.

Referral criteria

Referrals into the LAC Health Team are made by Social Workers for all looked after children and young people.

Who to contact if you need an interpreter or information about disabled access

Interpreters are arranged via the Social Worker in advance of appointments.
