Service description
CERT offers recovery focused interventions with people who need support to regain some of the skills which enable them to have a better quality of life in the community.
Services provided are positive and inclusive, flexible, individualised, accessible, and responsive.
CERT delivers time-limited, rehabilitation interventions within a community setting for service users who have severe and enduring mental health difficulties. Service users will be in the discharge process within an in-patient setting or in the community in their own accommodation, supported accommodation or residential accommodation.
There are three pathways within CERT:
Transition
We will support you during your transition from inpatient services into the community. We will start working with you during your inpatient stay when you are in the discharge process.
We will work with you for up to 12 weeks after transfer to the community. We aim to help reduce anxieties that arise when people are transferred from hospital and reduce the likelihood of readmission.
We work alongside your community mental health team (CMHT) to provide the support you need to adjust to living in the community.
We also provide support to your family and/or anyone else caring for you in the community. We can also cover the period in which you are awaiting allocation of a community psychiatric nurse (CPN) from your CMHT, to prevent your transfer from inpatient services being delayed.
Spoke
It is likely that you have been referred because your core mental health team feel you could benefit from a period of rehabilitation and recovery focused work in the community.
CERT will work with you for a period of time that is decided collaboratively between you, us and your community psychiatric nurse. We will decide together how often we see each other, tailored to your needs.
We will work together with you and your community mental health team to identify your rehabilitation goals and collaboratively plan your care to ensure we’re on track to meet these goals.
Hub
You have been referred to CERT for community support following transfer from inpatient services. It is likely that you have been referred as you would benefit from recovery focused rehabilitation interventions which can be provided in the community.
Within the “hub” element of CERT, you will be allocated a lead professional within the team who will support you in the community for around two years. We will be your primary community care team.
We will work collaboratively with you to identify your rehabilitation goals and collaboratively plan your care to ensure we’re on track to meet these goals within our two years of working together.
Referral criteria
Service users cannot self refer to CERT.
Referrals are accepted from secondary mental health services.
