Respiratory Service

Chronic Obstructive Pulmonary Disease (COPD) is the name for a collection of lung diseases including chronic bronchitis, emphysema and chronic asthma.

The Long Term Conditions (LTC) COPD Respiratory Service comprises a team of respiratory specialist nurses, respiratory specialist physiotherapists, oxygen specialist nurses and health care assistant support.

The team see housebound patients in their homes, provide respiratory clinics in our community hospitals and work alongside GPs and practice nurses to promote best practice in the management of Chronic Obstructive Pulmonary Disease (COPD). The aim of the service is to improve the care delivered to patients, reduce avoidable admissions to hospital, minimise length of stay, support self management and offer support and clinical advise to allied healthcare professionals. The team also deliver a telehealth service to suitable patients. The service can deliver education sessions to healthcare professional which includes inhaler technique training.

Referral criteria

Patients referred to the service must have a diagnosis of COPD confirmed by spirometry, and any of the following; a recent acute COPD exacerbation requiring hospital admission, a history of frequent COPD exacerbation, MRC of 3 or above, reaching end of life because of their COPD or would benefit from a specialist review.

To refer into the service please complete a LTC HUB referral form, e -referral or refer using PRISM.
