The community therapy service accepts referrals from a variety of different sources, including health care professionals and patient self referrals.

Once a referral is received it will be screened and triaged by a clinician who may call the patient or referrer for further information. Following this, the referral will be prioritised according to our referral criteria. Our service offers different response times including 2 hours, 2 days and 18 weeks, which will be determined by our triage clinicians based on individual needs.

The patient/carer will be contacted either by telephone or letter to advise of the expected outcome of the referral.

Referral criteria

Adults over the age of 18 registered with a GP in Leicester, Leicestershire or Rutland who require rehabilitation.

Criteria exclusion

Anyone not registered with a GP in Leicester, Leicestershire and Rutland and those under 17 years of age. If a referral is not appropriate for community therapy, the referrer will be signposted to the most appropriate service.

How to access this service

Patients/Carers

Patients can self-refer to the community therapy team by contacting SPA on 0300 300 7777

GPs

GPs will need to complete the PRISM referral form and make a e-referral to SPA via SystmOne or email securely to lptchs.spa@nhs.net

Other Healthcare Professionals  

Health professionals will need to contact the SPA healthcare professional line on 0300 300 1000.
