Adult Community Therapy Services

Our physiotherapy and occupational therapy teams help people regain or maintain function and independence in their daily life.

Our therapists treat people in their own homes, residential and nursing homes and in outpatient clinics. Following an initial assessment the therapy service will work with the patient to achieve mutually agreed treatment goals. The service actively promotes self management and patients are discharged from the service once their optimum level of function has been achieved. Community therapy has a specialist Falls Prevention Service for patients living in Leicestershire and Rutland.

City patients are able to access University Hospitals of Leicester’s Falls Prevention Service based at Leicester General Hospital. The City Falls Service is accessed via GP referral. Community therapy also has a specialist Community Integrated Neurology and Stroke Service (CINSS) for those patients with a neurological diagnosis that would benefit from specialist therapy intervention. The CINSS service works closely with speech and language therapists, neuropsychology and dietetics. In Leicester City our therapists also work within the Reablement Service (Social Care).

Referral criteria

This service is for adults registered with a GP in Leicester, Leicestershire or Rutland who require rehabilitation to support return of function and wellbeing to achieve the best health outcome.

Criteria exclusion

Anyone not registered with a LLR GP. Those under 17 years of age.

How to access this service

This service is for adults registered with a GP in Leicester, Leicestershire or Rutland who require rehabilitation to support return of their function and wellbeing.

To access our community therapy services you should speak to your GP or any other health or social care professional involved in your care eg. nurse, social worker. Once a referral is received it will be reviewed by a clinician and allocated to the most appropriate therapy team. At this point you will receive a letter advising you which team will treat you.
