Heart Failure Service
Heart failure is the name given when you are diagnosed with a problem with the pumping action of your heart. Our Long Term Conditions Heart Failure Specialist Service comprises of heart failure specialist nurses.
We see housebound patients in their homes, and provide specialist clinics in our community hospitals and health centres. We also work alongside GPs in providing expert knowledge, support and advice in the management and treatment of heart failure. The aim of the service is to improve the care delivered to patients, reduce avoidable admissions to hospital, minimise length of stay, support self management and offer support and clinical advice to healthcare professionals. The service works closely with the heart failure team within University Hospitals of Leicester. We provide support to newly diagnosed heart failure patients through to those with end stage illness where the focus may change to symptom control and palliative care.
Referral criteria
Patients referred to the community heart failure service must have a diagnosis of heart failure confirmed by an echocardiograph or MRI scan or angiograph. Patients with heart failure can be referred following a recent hospital admission for deteriorating heart failure or a worsening of their symptoms in the community which requires their medication to be titrated.

Heart failure secondary to left ventricular systolic dysfunction (whole of LLR) and heart failure with preserved ejection fraction (West CCG only).
