Community Hospitals
There are 13 wards situated in eight hospitals around Leicester, Leicestershire and Rutland, providing sub acute, complex rehabilitation and End of Life Care.
Ward 1 at St Luke’s Hospital and Ward 1 at Coalville Community Hospital are both stroke units caring for patients following the acute phase of a stroke.
Medical cover is provided by Advanced Nurse Practitioners (ANPs) with consultants visiting the wards twice weekly. Physiotherapy and occupational therapy is provided and all patients receive appropriate therapy assessments, including consideration of their psychological and cognitive needs, provision of goal-led intervention planned with the patient, together with holistic discharge planning. Care is planned, managed and overseen by registered staff with the support of unregistered assistants and technical instructors. Specialist occupational therapy and physiotherapy is provided at the two stroke units. Multidisciplinary care is provided in partnership with other professions and providers.
Adults requiring rehabilitation, palliative and end of life care or sub-acute care will be accepted onto the wards following a handover from the referring ward/GP. Admissions are accepted until 9pm.

Referral criteria
Referrals from University Hospitals of Leicester should be made via the Discharge Hub. All other referrals can be made through SPA tel: 0300 300 1000 (referrers only).
Adults requiring sub acute complex rehabilitation and end of life care will be accepted onto the wards following a handover from the referring ward/GP. This applies to all wards except Ward 1 at St Luke’s Hospital and Ward 1 at Coalville Hospital who specialise in specialist stroke rehabilitation only. Referrals to the specialist stroke wards will be accepted directly from University Hospitals of Leicester.
Criteria exclusion
Patients cannot be admitted for respite care. Patients requiring 24 hour medical care cannot be accepted.
