Diana Childrens Community Service - Short Break Care and Continuing Care Service

Our Diana Service offers families with children and young people with life limiting or life threatening conditions short breaks or continuing care.

Planned visits provide the opportunity for the family to have a break from the daily care routine of looking after their child/young person. Where the child or young person has complex healthcare needs, the visits aim to reduce the pressure on the family and help minimise the frequency of hospital admission.

To receive either level of service the child/young person’s healthcare needs will be formally assessed by a registered nurse in conjunction with parents/carers and a key professional (as appropriate). This assessment will then be submitted and reviewed. If the assessment is successful, the service will set up a package of care as the service capacity allows. An experienced pool of staff are available to provide care, but sometimes, the Diana service will employ further staff to be able to offer a family support.

Care is mainly provided by healthcare workers supported by a named Diana Nurse who will together manage and deliver the scheduled care package.

This care will mainly be provided from the home or within an appropriate community setting (as arranged).

Referral criteria

Children and young people up to 18 years old registered with a Leicester, Leicestershire or Rutland GP. A referral to receive short break care continuing care service can come from any professional (eg. health, social, education) involved with the child/young person. There is a set criteria for accepting referrals. If a child/young person is not eligible for either service, we will endeavour to signpost the family to other services who may be able to offer support.
