Family Nurse Partnership (FNP) Leicester City
Our FNP programme is an intensive home visiting programme for pregnant teenagers aged 19 and under.
The Family Nurse Partnership (FNP) is a voluntary programme for first time young mums aged 19 years and under. The family nurse works with the young mum from early pregnancy until their child is two. The named nurse visits the young mum at home, the programme aims to support mum, dad and their families in the transition to parenthood. The FNP programme aims to support in the areas of:
– A healthy pregnancy
– Improving their child’s health and development
– Plan their own futures and achieve their aspirations
The family nurses are all experienced nurses with the majority of them having a background in health visiting and midwifery. They have all completed extra training to become family nurses and practice within the core elements of the programme. The nurses have access to materials and activities that they can use when working with the mum, dad and wider family.
The programme works with the clients to build on their strengths and encourages them to fulfil their aspirations for their baby and themselves. There are weekly visits at the start of the programme, followed by fortnightly visits, leading onto monthly visits as the programme progresses. The FNP programme does not replace midwifery care but the nurses will lead and deliver on the Healthy Child Programme up until the child is two. At the age of two the child and mum will be transferred back to their local named public health nurse (formerly health visitor).
Referral criteria
Pregnant teenagers aged 19 years and under who live in Leicester City.
Referrals are accepted in the antenatal period up until 26 weeks of pregnancy.
